African American Family Reunion Program

The Diabetes Partnership of Cleveland will provide FREE information for you to give out at your next family reunion.

For people living in the Greater Cleveland area, contact us at 216-591-0800 with the date of the reunion and the number of families (not people) and children attending. We will assemble the information for you to pick up. Information cannot be mailed.

For those not living in the Greater Cleveland area, CLICK HERE to download and print the African American Family Reunion Kit, including information about diabetes and recipe cards.
